離校程序
Applying for Leaving School (For Undergraduates)
Step 1. 填寫系所問卷 / BME Questionnaire
登錄並填寫 醫工系離校 Google 表單，完成後請出示截圖給系辦確認。 Log in to the BME Leaving School Google Form and show a screenshot of the completed form to the department office for verification.
Step 2. 學校離校系統 / NCKU System
登錄 成大畢業生離校系統 並印出離校程序單。 Log in to the NCKU Leaving School System and print out the application form.
Step 3. 醫工系離校要求項目 / Department Requirements
以下本系要求之流程皆須完成 (Complete ALL the following requirements)：
- 繳交學士照照片： 請繳交 2 吋學士照 1 張，並於照片背面註明學號及姓名。
Submit one 2-inch graduation photo and write your student ID number and name on the back.
Step 4. 領取畢業證書 / Receive Diploma
按離校程序單上所列出的必要核章地點，依程序完成相關手續後即可領取畢業證書。 Bring the application form to the required divisions for approval and complete all leaving school procedures before collecting your diploma.
