Applying for Leaving School (For Postgraduates, Master & PhD)

Step 1

登錄填寫醫工系離校 Google 表單 【連結點我】
請出示完成表單的截圖給系辦確認。

Login the BME Graduation Questionnaire 【Link】

Step 2

登錄學校畢業生離校系統 【連結點我】 並印出離校程序單。

Login the NCKU Leaving School System 【Link】 and print out the application form.

Step 3

以下本系要求之流程皆須完成 Complete ALL the following requirements

1. 列印醫工系離校程序單 【下載】

   需由指導教授及實驗室負責人簽名後始得辦理。

   如借用本系或實驗室物品、置物櫃鑰匙或門禁卡， 離校前應歸還。

   Those who borrowed items, locker keys or access cards must return them before leaving NCKU.

2. 繳交碩 / 博士畢業照一張

   2 吋照片，背後請寫上學號與姓名

   Submit one graduation photo (2 inches). Write your name and student ID on the back.

3. 繳交論文紙本

   碩士：平裝一本 博士：精裝一本

   Submit one hard copy of your thesis/dissertation. Master: papercopy PhD: hardcover

4. 論文延後公開申請

   須於口試前透過 學位考試網路申請系統 提出申請。

   Application for embargo must be submitted through the Thesis Examination Online Application System before the oral defense.

   Only required for those who need to postpone thesis publication.

   • 涉及機密 Confidential information
   • 專利事項 Patent-related matters
   • 依法不得公開 Legally restricted disclosure
5. 指導教授同意離校證明書

   若有共同指導教授，皆需簽名。

   If there is a co-supervisor, both supervisors must sign.

6. 論文比對系統報告

   需提交最終版論文 Turnitin 報告頁， 並由指導教授簽名。

   Turnitin report of the final thesis version signed by your supervisor.

Step 4

依離校程序單完成各單位核章後， 即可領取畢業證書。

Bring the Leaving School Application Form to the required divisions for approvals to complete the process.
