Applying for Leaving School (For Postgraduates, Master & PhD)

Step 1. 登錄填寫醫工系離校Google 表單 【連結點我 】請出示已完成表單的截圖給系辦確認。

Step 2. 登錄學校畢業生離校系統  【連結點我 】  印出離校程序單
Login 【Online Application System for Graduation from NCKU】【Link 】 and print out the application form.

Step 3. 以下本系要求之流程皆須完成 Complete/Confirm ALL the following necessary requirements. 

1. 列印醫工系離校程序單  【連結點我 】 (醫工專用 / 由各實驗室指導教授及負責人簽名後, 始得辦理)
Print Graduation procedure form  【Link 】 (for BME department ONLY / Ask for the supervisor's approval (signature). Be submitted along with the Leaving school application form.) 
  ▲如借用本系或各實驗室物品、置物櫃鑰匙、門禁卡者，離校前應歸還相關物品。
  ▲Those who have borrowed items, locker keys, and access cards from the department or laboratories shall return the items before leaving NCKU.

2. 請繳交碩/博士照一張   (2吋/背後請寫上學號及姓名)
 Please submit one master's / doctoral graduation photo (2 inches / please write down your name & student ID on the back)

3. 碩士(平裝)或博士(精裝)論文一本
Submit one hard copy of your thesis/dissertation.  Masters: one hard copy in PAPERCOVER    PhD:  one hard copy in HARDCOVER

4. 論文延後公開申請：論文延後公開申請應於口試前於「學位考試網路申請作業」提出申請並完成作業，並於學位考試時提供延後公開相關事證，由所有考試委員審核確認後，於『學位論文延後公開申請書』上簽名。
Application for Embargo of Thesis/Dissertation: must be submitted and completed through the “Thesis Examination Online Application System” before the oral defense.
During the defense, supporting evidence for the request must be provided, and the application will be reviewed and confirmed by all committee members, who must sign the “Application Form for Delayed Thesis Publication.”
   ▲非必須，視需求而定，同時應符合下列申請要件：
   ▲NOT REQUIRED FOR ALL. For those who need to postpone the public access to their theses/dissertations ONLY, and must also meet the following requirements:
(1) 涉及機密：須提出適用法規或具體事實證據。
     Confidential information: Relevant regulations or concrete supporting evidence must be provided.
(2) 專利事項：須提供申請專利案號或提出相關申請說明。
     Patent-related matters: A patent application number or relevant application statement must be provided.
(3) 依法不得提供：須提出適用法規或具體事實證據。
     Legally restricted disclosure: Relevant regulations or concrete supporting evidence must be provided.

   ▲如果未於學位口試前提出申請，請至「學位考試網路申請網頁」申請異動，列印學位論文延後公開申請書，並經指導教授及所有考試委員簽名。同時須檢附延後公開的相關佐證文件。

If the application is not submitted before the oral defense, please apply for a modification through the “Thesis Examination Online Application System,” print out the Application Form for Delayed Thesis Publication, and obtain the signatures of your advisor and all committee members. Supporting documents for the delayed publication must also be attached.

5. 指導教授同意離校證明書 (若有共同指導者，皆需簽名)
"The Advisor's Consent for Graduation" (If there is a co-supervisor, both supervisors have to sign on the consent.)

6. 已上傳學校圖書館之完整版電子論文的論文比對系統報告頁。
Turnitin Report page of the Final version of your thesis/dissertation, your supervisor must sign for approval.

Step 4.按離校程序單上所列出的必要核章地點，依程序完成相關手續，領取畢業證書
Bring the Leaving School Application form to necessary divisions for approvals to complete the processing.
