離校程序
Applying for Leaving School (For Postgraduates, Master & PhD)
Step 1. 登錄填寫醫工系離校 Google 表單 【連結】
Login: Online Application System for Graduate Questionnaire from BME 【Link】
Step 2. 登錄學校畢業生離校系統 【連結】 並印出離校程序單
Login: Online Application System for Graduation from NCKU 【Link】 and print out the application form.
Step 3. 以下本系要求之流程皆須完成
Complete ALL the following necessary requirements.
1. 列印醫工系離校程序單 【連結】
Print Graduation procedure form 【Link】
如借用本系或實驗室物品、置物櫃鑰匙、門禁卡者，離校前應歸還相關物品。
Those who have borrowed items, locker keys, or access cards must return them before leaving NCKU.
2. 請繳交碩 / 博士照一張 (2 吋)
Please submit one master's / doctoral graduation photo (2 inches; write your name & student ID on the back)
3. 論文一本
Submit one hard copy of your thesis/dissertation.
Master: one paper cover copy
PhD: one hardcover copy
Application for Embargo of Thesis/Dissertation
Not required for all students. Only for those who need to postpone the public access of their thesis/dissertation.
4. 指導教授同意離校證明書
The Advisor's Consent for Graduation (If there is a co-supervisor, both must sign)
5. Turnitin Report
Turnitin report page of the final thesis/dissertation. Supervisor must sign for approval.
Step 4. 按離校程序單上所列出的必要核章地點完成相關手續並領取畢業證書。
Bring the Leaving School Application Form to necessary divisions for approval.
