Graduation procedure for Graduates

Step 1. 登錄填寫醫工系離校Google 表單 【連結點我 】

Step 2. 登錄學校畢業生離校系統  【連結點我 】  
Please link “Online Application System for Graduation from NCKU” to print out the "Graduation Procedure Form"

Step 3. 列印醫工系離校程序單  【連結點我 】 (醫工專用/由各實驗室指導教授及負責人簽名後,始得辦理)
Print Graduation procedure form  【Link 】 (just for BME/ Please be signed by the advisor and the person in charge of each laboratory before the submission)
►請繳交碩/博士照一張(2吋/背後請寫上學號及姓名)(當學期畢業審查已繳者可免繳)
 Please submit a master's/doctoral photo (2 inches / please write the student ID number and name on the back) (IF you had submitted it when applying for graduation review, no need to do it again)

►如借用本系或各實驗室物品、置物櫃鑰匙、門禁卡者,離校前應歸還相關物品。
Those who have borrowed items, locker keys, and access cards from the department or other laboratories should return the items before leaving the school.

►碩士(平裝)或博士(精裝)論文一本
One copy of your thesis/dissertation. For Master: Paper cover; For PhD: Hardcover

►指導教授同意離校證明書(若有共同指導者,皆需簽名)
" The Advisor's Consent for Graduation" (If there is a co-advisor, both advisors have to sign on the consent.)

►國家圖書館延後公開申請書(非必須;視需求而訂, 附者, 請夾在論文內第一頁內)
Authorization for Public Access of Thesis/ Dissertation (Not required; it's only for those who need to postpone the public access to their theses. The document should be attached behind the cover of the thesis)

Step 4.依程序完成相關手續
Complete the process from department to continue the process of graduation from university.
