離校程序

Applying for Leaving School (For Postgraduates, Master & PhD)

Step 1

登錄填寫醫工系離校 Google 表單：
Login to the BME Graduate Leaving School Google Form.

前往表單 / Open Google Form

Step 2

登錄學校畢業生離校系統並印出離校程序單：
Login to the NCKU Leaving School System and print out the application form.

Step 3

以下本系要求之流程皆須完成：
Complete ALL the following required procedures.

  1. 列印醫工系離校程序單
    Print the BME Leaving School Procedure Form.

    如借用本系或實驗室物品、置物櫃鑰匙、門禁卡者，離校前應歸還相關物品。
    Those who borrowed department or laboratory items, locker keys, or access cards must return them before graduation.

  2. 請繳交碩／博士照片一張（2 吋）
    Please submit one 2-inch graduation photo.
    Please write your name and student ID number on the back.
  3. 繳交論文一本
    Submit one hard copy of your thesis/dissertation.
    • Master: one paper-cover copy
    • PhD: one hardcover copy

    Application for Embargo of Thesis/Dissertation

    非所有學生皆須繳交，僅限需申請論文延後公開者。
    This application is only required for students who need to postpone public access to their thesis/dissertation.

  4. 指導教授同意離校證明書
    The Advisor’s Consent for Graduation.

    下載證明書 PDF / Download Form

    如有共同指導教授，需由雙方共同簽名。
    If there is a co-supervisor, signatures from both supervisors are required.

  5. Turnitin Report
    請繳交最終版論文之 Turnitin 比對報告首頁，並由指導教授簽名確認。
    Submit the Turnitin report page of the final thesis/dissertation with the supervisor’s signature.

Step 4

按離校程序單上所列之核章單位完成相關手續後，即可領取畢業證書。
Bring the Leaving School Application Form to the required divisions for approval and collect your diploma.

登入成功
